When to Use This
Send this as soon as you notice the fee. Acknowledging the mistake upfront disarms the reader. Mentioning your track record and the corrective steps you've taken makes the waiver feel low-risk for the company.
What Not to Say
Don't claim it wasn't your fault if it was. Avoid demanding the waiver like you're entitled to it. Don't threaten to switch providers over a single fee — save that leverage for bigger issues.
